MidSouth Transitions Medical Group
MidSouth Transitions Medical Group is a practice that provides house calls and telemedicine. We specialize in helping patients transition from in-patient medical care into long-term sustainability at home.
A myriad of procedures and tests can be administered in a patients home: injections, nebulizer treatments, lab work , wound management, X-Rays, IV fluids and more. In most cases, patients may self-refer, but some insurance companies require a referral.
We have a team of Physicians, Nurse Practitioners, Pharmacists, Dieticians, Nurse Case Managers, and additional team members ready to help patients in the comfort of their home.
